1. Complete the Coaches Information at the top of the Form

   • 13 Digit USSSA Registration Number (Located at the top of your Team Page)

2. Enter Players Information in Blue area's of the All-Star Nomination Form

3. Player may only be Nominated Once or the form will be REJECTED

4. MINIMUM of 3 Player's must be entered on the All-Star Nomination Form, or the form will be REJECTED

   1. College Players do not count against the 3 minimum

   2. Do Not Enter College Players on the All-Star Nomination Form

5. If the Player is Attending a College

   • Click the College All-Star Form Button

   • Enter Player Information

   • Enter the College that the Player is attending

DEADLINE IS

June 18, 2019

Forms that are not Filled In Correctly will be REJECTED

No Forms will be accepted after June 18, 2019
